A Renfrewshire nurse-led care home support team has been recognised at NHS Greater Glasgow and Clyde’s (NHSGGC) annual Celebrating Success Awards.

The care home nursing support team, which works across 23 older people's residential care homes in Renfrewshire, was awarded the gold prize in the nursing and midwifery category.

They were celebrated for their approach to preventing unnecessary hospital admissions from care homes.

In the year to the end of March, the support team has conducted direct assessments of 772 residents within these care homes.

These assessments resulted in a total of 3,048 consultations with only eight per cent leading to a referral to the acute services of the NHS.

They also provided 68 training sessions for staff which covered a range of clinical topics.

Meanwhile, the stroke multidisciplinary team at the Royal Alexandra Hospital secured the overall award within the Clyde sector local category.

This was in recognition of their work in establishing a thrombolysis service at the hospital.

This service, if accessed quickly, can help to improve outcomes for stroke patients, and having it at the the Royal Alexandra Hospital means people no longer have to be transferred to the Queen Elizabeth University Hospital.

In addition, the delayed discharge team was recognised as the overall winner of the Renfrewshire health and social care partnership category.

This team collaborates closely with acute services to ensure patients receive the necessary community-based support after they are discharged from the hospital.

The event which took place at the Radisson hotel in Glasgow, presented awards to teams and individuals across seven Excellence Award categories.

A total of 18 winners were announced, spanning NHSGGC's local sectors, health and social care partnerships, and other services including diagnostics, public health and pharmacy.
